Abstract

With the globalization of healthcare infrastructure, there has been an exponential growth of data exchange between different borders for telemedicine. The legal and technical structures that enable these transfers, however, are highly fractured, which leads to new information security governance and patient privacy challenges. In this paper, the authors examine the gap between the systems of healthcare communication technologies and the compliance matrices of international jurisdictions. With the help of a critical synthesis of the existing data security governance frameworks, we present a design for a three-tiered socio-technical model: Cross-Border Telemedicine Governance Architecture, which incorporates cryptographic compliance, decentralized ledger validation, and adaptive multi-jurisdictional policy mapping. The paper is based on two theories: Institutional Anomie and Socio-Technical Systems (STS), which have three testable research propositions that are presented: technical policy automation as a mediator and inter-organizational trust as a moderator. A complete specification matrix of the Structural Equation Modeling (SEM) is developed to help with empirical validation. Last, roadmaps are offered that can be applied to a healthcare administrator and system engineer who are attempting to manage a world-wide data environment.
